Revascularized autotransplants (iliac crest, fibula free flaps) are used to reconstruct defects of the mandible and restore joint function.</p><p><bold>Aim.</bold> To evaluate functional, esthetic and oncological results of fragmental mandibular resection with exarticulation and single-step reconstruction using bone flaps.</p><p><bold>Materials and methods.</bold> The study included 52 patients who underwent segmental mandibular resection with exarticulation and single-step reconstruction using bone flaps. For defects of the ramus and head of the mandible, reconstruction was performed using iliac crest free flap (5 cases); defects of the ramus and body of the mandible, fibula free flap was used (47 cases).</p><p><bold>Results.</bold> Total necrosis of the flap was observed in 1 (4.7 %) case. Iliac crest free flap was modelled taking into account the size of mandibular ramus defect (usually to the angle of the mandible), mandibular head was formed from the distal part. At least one osteotomy was preformed on the autotrasplant near the angle, on the distal part the head was formed. Follow-up period varied between 1 and 5 years. Disease progression in the form of recurrence was observed in 15 (34.9 %) cases, in the form of distant metastases in 6 (13.9 %) cases.</p><p><bold>Conclusion. </bold>For small defects of the ramus of the mandible, revascularized iliac crest free flap should be used, for large defects the method of choice is fibula free flap. Temporomandibular joint can be formed from the distal end of a bone autotransplant with its subsequent adaptation during functional load.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Выполнение сегментарной резекции нижней челюсти с экзартикуляцией и одномоментной реконструкцией является одной из актуальных проблем черепно-челюстно-лицевой хирургии. Для ее решения применяются реваскуляризированные аутотрансплантаты (подвздошный, малоберцовый лоскуты), что позволяет выполнить замещение дефекта нижней челюсти и восстановить функцию сустава.</p><p><bold>Цель исследования </bold>– оценить функциональные, эстетические и онкологические результаты фрагментарной резекции нижней челюсти с экзартикуляцией и одномоментной реконструкцией костными лоскутами.</p><p><bold>Материалы и методы.</bold> В исследование включены 52 пациента, которым выполнена сегментарная резекция нижней челюсти с экзартикуляцией и одномоментной реконструкцией костными лоскутами. Если замещался только дефект ветви и суставной головки нижней челюсти, для реконструкции использовали подвздошный лоскут (5 случаев), при дефекте ветви и тела нижней челюсти – малоберцовый лоскут (47 случаев).</p><p><bold>Результаты.</bold> Тотальный некроз лоскута отмечен в 1 (4,7 %) случае. Подвздошный аутотрансплантат моделировался с учетом размера дефекта ветви нижней челюсти (чаще всего до угла нижней челюсти), суставная головка формировалась из дистальной части. На малоберцовом аутотрансплантате выполнялась как минимум одна остеотомия в области угла нижней челюсти, на дистальной части аутотрансплантата формировалась суставная головка. Срок наблюдения за пациентами составил от 1 года до 5 лет. Прогрессирование заболевания в виде рецидива отмечено в 15 (34,9 %) случаях, в виде отдаленного метастазирования – в 6 (13,9 %).</p><p><bold>Заключение. </bold>При небольших дефектах ветви нижней челюсти целесообразно использовать реваскуляризированный подвздошный лоскут, при протяженном дефекте методом выбора является малоберцовый лоскут.
