In experimental models, this drug has demonstrated nanomolar activity against various <italic>RET </italic>gene alterations, including an acquired resistance mutation in the gatekeeper gene at residue V804, as well as antitumor activity in the brain. Selpercatinib inhibits wild-type and various mutated forms of RET, as well as vascular endothelial growth factor receptors (VEGFR1 and VEGFR3) with half-maximal inhibitory concentrations from 0.92 to 67.8 nM, and fibroblast growth factor receptors (FGFR1, FGFR2, and FGFR3).</p><p>The article presents results of clinical studies on the efficacy and safety of selpercatinib in patients with radioiodine refractory differentiated thyroid cancer with alterations in <italic>RET </italic>gene and in patients with medullary thyroid cancer with a germinal or somatic mutation in this gene.</p></abstract><trans-abstract xml:lang="ru"><p>Селперкатиниб – инновационный высокоселективный низкомолекулярный ингибитор RET-киназы, конкурирующий с аденозинтрифосфорной кислотой (АТФ). В экспериментальных моделях этот препарат продемонстрировал наномолярную активность против различных альтераций гена <italic>RET</italic>, в том числе приобретенной резистентной мутации в гене-привратнике в области остатка V804, а также противоопухолевую активность в головном мозге. селперкатиниб ингибирует RET-рецептор «дикого типа» и различные мутированные формы RET-рецептора, а также рецепторы фактора роста эндотелия сосудов (VEGFR1 и VEGFR3) с концентрацией полумаксимального ингибирования от 0,92 до 67,8 нМ и рецепторы фактора роста фибробластов (FGFR1, FGFR2 и FGFR3).</p><p>В статье представлены результаты клинических исследований эффективности и безопасности селперкатиниба у пациентов с радиойодрефрактерным дифференцированным раком щитовидной железы с перестройками в гене <italic>RET </italic>и у пациентов с медуллярным раком щитовидной железы с герминальной или соматической мутацией в этом гене.</p></trans-abstract><kwd-group xml:lang="en"><kwd>medullary thyroid cancer</kwd><kwd>radioiodine refractory thyroid cancer</kwd><kwd>progression</kwd><kwd>calcitonin</kwd><kwd>RET mutation</kwd><kwd>targeted therapy</kwd><kwd>selpercatinib</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>медуллярный рак щитовидной железы</kwd><kwd>радиойодрефрактерный рак щитовидной железы</kwd><kwd>прогрессирование</kwd><kwd>кальцитонин</kwd><kwd>RET-мутация</kwd><kwd>таргетная терапия</kwd><kwd>селперкатиниб</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Wells S.A.
