If there is a violation of the full replenishment of the need for nutrients in a natural way, the need for longterm nutritional support at the stages of antitumor treatment and the preservation of the function of the gastrointestinal tract, the method of choice is the formation of a gastrostomy. In addition to the effects of its direct purpose, gastrostomy promotes social adaptation of patients, as well as eliminates the risks associated with the installation and prolonged use of a nasogastric tube, and is compatible with other surgical procedures at the stages of antitumor treatment, symptomatic and palliative care.</p><p><bold>Aim.</bold> To evaluate the clinical and social significance of performing gastrostomy in patients with squamous cell carcinoma of the head and neck.</p><p><bold>Materials and methods.</bold> The present study included 150 patients with squamous cell carcinoma of the head and neck organs who underwent minimally invasive gastrostomy (133 / 150; 88.7 %), open-label (11 / 150; 7.3 %) or interventional under CT control (6 / 150; 4 %). In 88 (58.7 %) patients, gastrostomy was combined with other medical procedures (tumor biopsy, tracheostomy, selective intraarterial embolization). Descriptive statistics methods and the t-test were used to evaluate the results of the study. The differences were considered significant at <italic>p</italic> &lt;0.05.</p><p><bold>Results.</bold> Postoperative complications were observed in 12.7 % of patients, hospital mortality was 2 %. The average length of stay of patients in the hospital with uncomplicated course of the postoperative period was 7.1 ± 5.0 days, in the presence of complications – 18.2 ± 18.5 days. Among patients receiving antitumor treatment after gastrostomy drainage, the median time between gastrostomy and its onset was 20 days, and the median overall survival was 261.5 days. for patients under medical supervision, the median overall survival reached 621 days. This indicator in patients who did not receive antitumor treatment after gastrostomy and patients referred to the palliative Care Center was 52 and 61 days, respectively.</p><p><bold>Conclusion. </bold>performing gastrostomy in a minimally invasive manner is an essential component of the prehabilitation of debilitated patients with complicated squamous cell carcinoma of the head and neck organs. This manipulation allows you to start or resume antitumor treatment, as well as increase overall survival.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> У многих пациентов со злокачественными новообразованиями органов головы и шеи имеется белковоэнергетическая недостаточность, что связано с локализацией опухоли в непосредственной близости к проксимальному отделу пищеварительного тракта, ее агрессивным ростом, наличием осложнений опухолевого процесса, а также проведением лекарственного и / или лучевого лечения. при нарушении полноценного восполнения потребности в нутриентах естественным путем, необходимости в длительной нутритивной поддержке на этапах противоопухолевой терапии и сохранении функции желудочно-кишечного тракта методом выбора является формирование гастростомы. помимо своего прямого действия гастростомия способствует социальной адаптации больных, а также исключает риски, связанные с установкой и длительным использованием назогастрального зонда. Она совместима с другими хирургическими манипуляциями на этапах противоопухолевого лечения, а также оказания симптоматической и паллиативной помощи.</p><p><bold>Цель исследования </bold>– оценка клинической и социальной значимости хирургической коррекции нутритивного статуса путем выполнения гастростомии у пациентов с плоскоклеточным раком органов головы и шеи.</p><p><bold>Материалы и методы. </bold>В исследование включены 150 пациентов с плоскоклеточным раком органов головы и шеи, которым в 2020–2025 гг. в отделении опухолей головы и шеи Онкологического центра № 1 городской клинической больницы им. с. с. Юдина выполнена гастростомия малоинвазивным доступом (133 / 150; 88,7 %), открытая (11 / 150; 7,3 %) или интервенционная под контролем компьютерной томографии (6 / 150; 4 %). у 88 (58,7 %) пациентов гастростомия сочеталась с другими медицинскими манипуляциями (биопсией опухоли, трахеостомией, селективной внутриартериальной эмболизацией). Для оценки результатов использованы методы описательной статистики и <italic>t</italic>-критерий стьюдента. Различия считались статистически значимыми при <italic>р</italic> &lt;0,05.</p><p><bold>Результаты. </bold>послеоперационные осложнения наблюдались у 12,7 % больных. госпитальная летальность составила 2 %, среднее время нахождения пациентов в стационаре при неосложненном течении послеоперационного периода – 7,1 ± 5,0 койко-дней, при наличии осложнений – 18,2 ± 18,5. у пациентов, получавших противоопухолевое лечение после формирования гастростомы, медиана времени между его началом и гастростомией оказалась равной 20 дней, медиана общей выживаемости – 261,5 дня. у больных, находящихся на диспансерном наблюдении, медиана общей выживаемости достигла 621 день. Данный показатель у пациентов, не получающих противоопухолевое лечение после гастростомии, и больных, направленных в Центр паллиативной помощи, составил 52 и 61 день соответственно.</p><p><bold>Заключение.</bold> Выполнение гастростомии малоинвазивным способом является важнейшим компонентом преабилитации ослабленных больных с осложненным течением плоскоклеточного рака органов головы и шеи.
