Исследование SELECT, посвященное изучению эффективности ленватиниба (E7080) в лечении дифференцированного рака щитовидной железы, продемонстрировало улучшение показателей выживаемости без  прогрессирования (ВБП) по  сравнению с плацебо у пациентов с радиойодрефрактерным дифференцированным раком щитовидной железы (РР ДРЩЖ). В рамках данного анализа проведена оценка влияния возраста пациентов на эффективность и безопасность применения ленватиниба.</p><p><bold>Материалы и методы</bold>. В данное рандомизированное двойное слепое исследование III фазы включены пациенты с гистологически подтвержденным РР ДРЩЖ, которые были стратифицированы по возрасту (≤65 и &gt;65 лет). Пациенты (N = 392) получали ленватиниб в дозе 24 мг/сут (n = 261) или плацебо (n = 131). В качестве первичной конечной точки была принята ВБП; в качестве вторичных конечных точек – общая выживаемость (ОВ), частота объективного ответа и безопасность.</p><p><bold>Результаты</bold>. В обеих группах медианы возраста составили 56 лет (в подгруппе более молодых пациентов) и 71 год (в подгруппе пожилых пациентов). Преимущество ленватиниба по показателю ВБП в сравнении с плацебо было продемонстрировано в обеих подгруппах: медиана ВБП составила 20,2 против 3,2 мес у более молодых пациентов (отношение рисков (ОР) 0,19; 95 % ДИ 0,13–0,27; p = 0,001) и 16,7 против 3,7 мес – у пожилых пациентов (ОР 0,27; 95 % ДИ 0,17–0,43; p = 0,001). ВБП не была ассоциирована с возрастом пациентов в обеих группах. ОВ у пожилых пациентов, получавших ленватиниб, была выше, чем у пациентов той же возрастной подгруппы, получавших плацебо (ОР 0,53; 95 % ДИ 0,31–0,91; р = 0,020). У более молодых пациентов, получавших ленватиниб, по сравнению с пожилыми пациентами зарегистрирована статистически значимо более высокая частота объективного ответа (72 % против 55 %; р = 0,0038), 1-я редукция дозы требовалась в более поздние сроки (3,7 против 1,5 мес) и нежелательные явления III–V степени тяжести наблюдались в меньшем числе случаев (67 % против 89 %; р = 0,001).</p><p><bold>Заключение</bold>. Данный субанализ продемонстрировал улучшение ВБП при терапии ленватинибом по сравнению с плацебо в обеих возрастных подгруппах, хотя токсические реакции чаще наблюдались у пожилых пациентов.
