Thirty-three stage III—IV oral cancer patients aged between 39 and 70 years were asked to fill EORTC QLQ-C30 (European Organisation for Research and Treatment of Cancer quality of life questionnaire core 30-questions) and QLQ-H&amp;N35 (head and neck cancer-specific module) before and 12—18 months after completion of the treatment that consisted of surgery followed by radiation with or without chemotherapy. Surgery included neck dissection with removal of the primary tumor via transoral (n = 5) or combined (n = 28) approach. Reconstruction was performed by primary closure (n = 5), with pedicled (n = 8) or free (n = 20) flaps.</p><p><bold>Results.</bold> According to EORTC QLQ-C30 questionnaire only positive shifts turned out to be statistically significant: general health, emotional function, pain, insomnia and diarrhea. Site-specific EORTC QLQ-H&amp;N35 questionnaire revealed several positive (pain in the head and neck, feeling ill, use of painkillers and weight gain) and negative (public eating, problems with taste and smell, sticky saliva and dry mouth) changes. Applying algorithms for determining clinical significance changed the number and value of several scales and domains. Changes in general health, emotional function, pain, insomnia, pain in the head and neck, taste and smell disorders, mouth opening, sticky saliva, dry mouth, painkillers and weight gain were found to have some clinical relevance. Moreover, for one of them (mouth opening) statistical significance was not reached.</p><p><bold>Conclusions.</bold> Further research of clinical significance of changes and differences in scales and domains that determine and affect quality of life are needed. They will allow to understand more fully problems that every patient with oral cavity cancer tries to cope with.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования</bold> — изучить качество жизни (КЖ) пациентов с плоскоклеточным раком головы и шеи как один из основных критериев эффективности их лечения.</p><p><bold>Материалы и методы</bold>. Проанализированы результаты заполнения опросников EORTC QLQ-C30 (European Organisation for Research and Treatment of Cancer quality of life questionnaire core 30-questions) и QLQ-H&amp;N35 (head and neck cancer-specific module) 33 пациентами в возрасте от 39 до 70лет с раком полости рта III—IVстадий, у которых было проведено хирургическое лечение с последующей адьювантной лучевой или химиолучевой терапией. Хирургический этап лечения включал шейную диссекцию, удаление первичной опухоли с использованием чрезротового (n = 5) или комбинированного доступа (n = 28). Устранение дефекта осуществляли с применением лоскутов с осевым кровоснабжением (n = 8) или с помощью микрохирургической аутотрансплантации (n = 20). В 5 случаях дополнительный пластический материал не использовали. До начала лечения и через 6—12 мес после него при отсутствии прогрессирования заболевания проводили анкетирование пациентов с помощью опросников EORTC QLQ-C30 и QLQ-H&amp;N35.</p><p><bold>Результаты.</bold> По опроснику EORTC QLQ-C30 отмечена положительная динамика следующих параметров КЖ: общего состояния здоровья, эмоционального функционирования, общего уровня боли, частоты бессонницы и диареи. Оценка по опроснику EORTC QLQ-H&amp;N35 выявила разнонаправленные изменения. Улучшение КЖ наблюдалось по параметрам уровня боли в области головы и шеи, частоты применения анальгетиков, интенсивности ощущения себя больным и степени набора массы тела. КЖ ухудшилось по шкалам выраженности затруднений при публичном приеме пищи, нарушения восприятия вкуса и запаха, наличия вязкой слюны и ксеростомии. Функция глотания имела лишь тенденцию к ухудшению. После применения известных алгоритмов интерпретации изменений КЖ клинически значимыми оказались изменения по параметрам общего состояния здоровья, эмоционального функционирования, общего уровня боли, частоты бессонницы, уровня боли в области головы и шеи, выраженности нарушения восприятия вкуса и запаха, затруднений при открывании рта, наличия вязкой слюны, ксеростомии, частоты приема анальгетиков и степени набора массы тела, причем изменения по шкале выраженности затруднений при открывании рта не достигли уровня статистической значимости.</p><p><bold>Заключение.</bold> Дальнейшее изучение клинической значимости изменений параметров и шкал КЖ, оцениваемых с использованием опросников различного типа, позволило бы составить более ясное представление о степени благополучия пациентов, страдающих от рака полости рта.</p></trans-abstract><kwd-group xml:lang="en"><kwd>oral cancer</kwd><kwd>quality of life</kwd><kwd>surgery</kwd><kwd>radiotherapy</kwd><kwd>chemotherapy</kwd><kwd>EORTC QLQ-C30</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>рак полости рта</kwd><kwd>качество жизни</kwd><kwd>хирургическое лечение</kwd><kwd>лучевая терапия</kwd><kwd>химиотерапия</kwd><kwd>EORTC QLQ-C30</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">1.
