In the period from 2012 to 2016, 182 patients with relapses of squamous cell carcinoma of the larynx, oral cavity and tongue received treatment in the Chelyabinsk regional clinical center of Oncology and nuclear medicine. The group 1 included 66 patients with resectable relapse who were operated. The group 2 consisted of 25 patients who received a course of radiation therapy. The group 3 consisted of 46 patients who underwent chemotherapeutic treatment of relapse. The group 4 was represented by combined treatment (surgery + radiation therapy), this group included 9 people. The group 5 consisted of those of patients who were not specifically treated for relapse because of the low Karnofsky index in patients (&lt;70 %).</p><p><bold> Results.</bold> OS among patients who received special treatment was significantly higher compared with the group of symptomatic therapy. The median OS in the special treatment group was 40 months, and without it – 18 months. Comparing all types for treating relapse, the highest rates of OS were in the surgical treatment group. OS rates in combined therapy group and radiotherapy group were comparable rate in the radiotherapy. The lowest OS rate was after chemotherapy (only 21 months).</p><p><bold>Conclusion.</bold> Surgery is the optimal method for treating recurrent squamous cell carcinoma of head and neck, if it is resectable. If surgical treatment is not possible, no other method significantly increases the OS. If the overall status of the patient is normal, re-radiation or polychemotherapy may be performed. A limited category of patients can be subjected to combined treatment for relapse. </p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> В настоящее время отсутствуют методы лечения рецидивов плоскоклеточного рака головы и шеи, достоверно увеличивающие общую выживаемость (ОВ) пациентов, что обусловливает актуальность данной проблемы.</p><p><bold> Цель исследования</bold> – анализ результатов лечения пациентов с рецидивами плоскоклеточного рака головы и шеи, оценка влияния факторов риска рецидива и различных вариантов лечения рецидива на показатели ОВ.</p><p><bold> Материалы и методы.</bold> Проанализированы данные 182 больных с рецидивами плоскоклеточного рака гортани, полости рта и языка, находившихся на лечении в Челябинском областном клиническом центре онкологии и ядерной медицины в период с 2012 по 2016 г. В 1-ю группу вошли 66 пациентов, подвергшихся хирургическому лечению, во 2-ю – 25 пациентов, прошедших курс повторной лучевой терапии, в 3-ю – 46 пациентов, у которых проведена полихимиотерапия, в 4-ю – 9 пациентов, лечение которых было комбинированным, в 5-ю – 36 пациентов, которым специальное лечение не было назначено ввиду низкого индекса Karnofsky (&lt;70 %).</p><p><bold> Результаты.</bold> ОВ после специального лечения оказалась статистически значимо выше, чем после симптоматической терапии. Медианы ОВ пациентов, проходивших специальное лечение и не проходивших его, составили соответственно 40 и 18 мес. При оценке зависимости ОВ от факторов риска рецидива выявлена тенденция к увеличению ОВ у пациентов с отсутствием факторов риска. Наиболее высокие показатели ОВ зарегистрированы после хирургического лечения. После комбинированного лечения и лучевой терапии показатели ОВ оказались сопоставимыми. Наиболее низкая ОВ наблюдалась после химиотерапии – 21 мес.</p><p><bold> Заключение.</bold> Хирургическое вмешательство – оптимальный метод лечения рецидивов плоскоклеточного рака головы и шеи при условии их резектабельности. При невозможности хирургического лечения ни один другой метод достоверно не увеличивает ОВ. При удовлетворительном общем статусе пациента возможно провести повторное облучение или полихимиотерапию.
