In 29 cases the tumor was resected transorally (Group 1), in 45 – <italic>via</italic> combined approach (Group 2). Group 2 patients had more advanced tumors both locally and regionally. 13 patients of Group 1 and 27 patients of Group 2 underwent adjuvant radiotherapy. The following parameters were used for comparative analysis: the rate of local and regional recurrence, locoregional control, the rate of distant metastasis and Kaplan–Meyer overall survival.</p><p><bold>Results. </bold>Mean follow up was 33.77 ± 27.72 months (range 14–115 months). The rate of local and regional recurrence was higher in Group 2 (20 % <italic>vs </italic>10.3 % and 22.2 % <italic>vs </italic>17.2 % respectively). Locoregional control was better in Group 1 (72.4 % <italic>vs</italic> 62.2 %). The difference for neither of the above-mentioned parameters did not reach statistical significance. Median survival was statistically significantly better in Group 1: 66 ± 17.42 months <italic>vs </italic>23 ± 3.85 months (<italic>p</italic> = 0.030). Poorer treatment results in Group 2 can be explained by a higher proportion of patients with more advanced tumors in this group.</p><p><bold>Conclusion.</bold> The results of the present study do not allow to conclude that combined approach has oncologic advantage over less aggressive transoral approach for tumors with depth of invasion from 10 to 20 mm. It is questionable to recommend the combined approach as a universal one for lesions with such a stage of local invasiveness.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Хирургический доступ при раке полости рта является одним из ключевых моментов, определяющих онкологическую эффективность лечения, риск развития осложнений в послеоперационном периоде, необходимость использования дополнительного пластического материала и т. д. В настоящее время отсутствуют жесткие стандарты применения того или иного вида доступа, базирующиеся на строго научных данных при определенной глубине инвазии первичной опухоли.</p><p><bold>Цель исследования </bold>– ретроспективный сравнительный анализ онкологической эффективности чрезротового и комбинированного доступов в хирургическом лечении рака полости рта умеренной степени местного распространения.</p><p><bold>Материалы и методы.</bold> В исследование включены 75 пациентов в возрасте от 30 до 80 лет с плоскоклеточным раком языка и дна полости рта с глубиной инвазии от 10 до 20 мм, 29 из которых удаление опухоли полости рта проводилось чрезротовым (1-я группа), а 45 – комбинированным (2-я группа) доступом. Для пациентов 2-й группы была характерна большая степень местного и регионарного распространения опухолевого процесса. Адъювантная лучевая терапия проведена 13 пациентам 1-й и 27 пациентам 2-й группы. Онкологическая эффективность сравнивалась с помощью следующих параметров: частота местного и регионарного рецидивов, отдаленного метастастазирования, локорегионарный контроль, общая выживаемость по методу Каплана–Майера.</p><p><bold>Результаты. </bold>Средний срок наблюдения составил 33,77 ± 27,72 мес (диапазон 14–115 мес). Частота местного и регионарного рецидивов и отдаленного метастазирования оказалась выше во 2-й группе по сравнению с 1-й группой (20 % против 10,3 %; 22,2 % против 17,2 %; 11,1 % против 6,9 % соответственно), а локорегионарный контроль – в 1-й группе (72,4 % против 62,2 %). Ни для одного из вышеприведенных параметров разница не достигла уровня статистической значимости. Медиана общей выживаемости была статистически достоверно выше в 1-й группе: 66 ± 17,42 мес против 23 ± 3,85 мес (<italic>р</italic> = 0,030). Худшие результаты лечения пациентов 2-й группы могут быть объяснены большей степенью распространенности опухолевого процесса.</p><p><bold>Заключение. </bold>Полученные результаты не позволяют сделать вывод о преимуществе комбинированного доступа над менее агрессивным чрезротовым при раке языка и дна полости рта средней степени местного распространения с глубиной инвазии от 10 до 20 мм.
