Due to the characteristics of dose distribution, proton beam therapy is a promising treatment option for patients with recurrences of tumors in this location who previously underwent radiation therapy.</p><p><bold>Aim. </bold>To evaluate the effectiveness and tolerability of repeat irradiation using active scanning proton beam therapy in patients with recurrent head and neck tumors who previously underwent radiation therapy.</p><p><bold>Materials and methods. </bold>Between November of 2015 and December of 2020, 40 patients with locoregional recurrence of head and neck tumors underwent treatment using active scanning proton beam therapy at the A. F . Tsyb Medical Radiological Research Center – branch of the National Medical Research Radiological Center. Median cumulative dose of primary irradiation was 64.5 Gy. Median time between primary and repeat irradiation was 35.7 months, mean irradiated volume of the repeat course was 94.5 cm<sup>3</sup>. Proton beam therapy was performed using standard mode (2 isoGy) and accelerated hypofractionation (2.4 isoGy / 3 isoGy) with mean equivalent cumulative dose of 56.4 Gy (α / β = 10). Radiation toxicity was evaluated using the Radiation Therapy Oncology Group European (RTOG) / Organization for Research and Treatment of Cancer (EORTC) scale.</p><p><bold>Results. </bold>Treatment response was achieved in 34 (85 %) patients: in 17 (42.5 %) patients, stable disease was observed; in 10 (25 %) patients, partial response was observed; and in 7 (17.5 %) patients, complete response was observed. In 6 (15 %) cases, disease progression was diagnosed at first follow-up examination. One- and two-year locoregional control, progression-free survival and overall survival were 58.4 / 19.8; 44.5 / 19.8 and 82.3 / 38.8 % respectively with median follow-up duration of 14.2 months. Median survival was 19.5 months. Grade III and above early radiation toxicity was observed in 3 (7.5 %) patients. In total, 6 (15 %) cases of grade III complications and 2 (5 %) episodes of carotid artery rupture leading to death were observed. Overall frequency of complications of grade III and higher was 20 %.</p><p><bold>Conclusion. </bold>Repeat irradiation using proton beam therapy can be considered an effective and safe treatment method for patients with recurrent head and neck tumors. Dosimetric and radiobiological benefits of proton beams allow to achieve balance between high doses and radiation exposure in previously irradiated tissues.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Рецидивирование опухолей головы и шеи возникает в 50 % случаев и наиболее часто носит локорегионарный характер. В связи с особенностями распределения дозы протонная терапия может быть перспективной опцией лечения пациентов с рецидивами опухолей этой локализации, которым ранее проводилась лучевая терапия.</p><p><bold>Цель исследования </bold>– оценка эффективности и переносимости повторного облучения методом протонной терапии активным сканирующим пучком пациентов с рецидивными опухолями головы и шеи, которым ранее проводилась лучевая терапия.</p><p><bold>Материалы и методы. </bold>С ноября 2015 по декабрь 2020 г. в Медицинском радиологическиом научном центре им. А. Ф . Цыба – филиале ФГБУ «Национальный медицинский исследовательский центр радиологии» Минздрава России с помощью методики протонной терапии активным сканирующим пучком прошли лечение 40 пациентов с локорегионарным рецидивом опухолей головы и шеи. Медиана суммарной дозы первичного облучения была равна 64,5 Гр. Медиана интервала между первым и повторным облучением составила 35,7 мес, средний облучаемый объем повторного курса – 94,5 см<sup>3</sup>. Протонная терапия проводилась в традиционном режиме (2 изоГр) и в режимах ускоренного гипофракционирования (2,4 изоГр / 3 изоГр) до средней эквивалентной суммарной дозы в 56,4 Гр (α / β = 10). Токсичность облучения оценивалась по шкале Radiation Therapy Oncology Group European (RTOG) / Organization for Research and Treatment of Cancer (EORTC).</p><p><bold>Результаты. </bold>Ответа на лечение удалось достичь у 34 (85 %) пациентов: у 17 (42,5 %) наблюдалась стабилизация заболевания, у 10 (25 %) – частичный и у 7 (17,5 %) – полный ответы. В 6 (15 %) случаях отмечено прогрессирование заболевания при первом контрольном обследовании. Показатели одно- и двухлетнего локорегионарного контроля, выживаемости без прогрессирования и общей выживаемости составили 58,4 / 19,8; 44,5 / 19,8 и 82,3 / 38,8 % соответственно при медиане наблюдения в 14,2 мес. Медиана выживаемости была равна 19,5 мес. Явления ранней лучевой токсичности III степени и выше отмечены у 3 (7,5 %) пациентов. Выявлены 6 (15 %) случаев поздних осложнений III степени и 2 (5 %) эпизода разрыва сонной артерии, приведших к фатальному исходу. Общая частота возникновения осложнений III степени и выше составила 20 %.</p><p><bold>Заключение. </bold>Повторное облучение с использованием протонной терапии может считаться эффективной и безопасной методикой лечения пациентов с рецидивами опухолей органов головы и шеи.
