However, survival rates are still relatively unchanged. The surgical stage of treatment leads to extensive defects that are eliminated through reconstructive technologies. Despite the priority of performing reconstruction at the same time, delayed reconstructions are still being discussed 6–12 months after surgical treatment; therefore, it is necessary to develop new methodological and practical approaches.</p><p><bold>Aim. </bold>To determine the features of the delayed reconstructive-plastic stage in patients with malignant tumors maxillofacial region, evaluation of possible solutions for optimizing the stage.</p><p><bold>Material and methods. </bold>The analysis of available literature sources was taken in the database Medline, Pubmed, eLibrary, etc. The 101 studies were found, 60 were used to write a systematic review.</p><p><bold>Results.</bold><bold> </bold>Delayed reconstructive plastic surgery should take into the possibility more complex and larger defects of soft and bone tissues during the reconstructive stage, previous surgery and/or radiation therapy create significant difficulties for the identification of recipient vessels. Computer-aided design (CAD)/computer-aided manufacturing (CAM) technologies allow projecting the design and positioning of reconstructive material at the preoperative stage. Mandibular reconstruction is the most difficult, that isn’t achieve only an aesthetic result, but also to restore the biomechanics of the temporomandibular joint. The development of specific complications hinders the improvement of the patient’s quality of life. In the early postoperative period is may develop necrosis of the flap, thrombosis of vascular pedicle, hematoma; in the late postoperative period plate extrusion is still one of the common complications, planning delayed reconstruction an important aspect is prevention or treatment of osteoradionecrosis. The combination of precarbohydrate loading and dalargin will allow to reduce the frequency of perioperative complications and improve the long-term results of surgical treatment.</p><p><bold>Conclusion. </bold>Reconstructive treatment in patients with defects requiring postponed reconstructive surgeries is a complex problem which requires development of an integrated approach with detailed analysis of the existing defect and previous antitumor therapy.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Основным вариантом лечения злокачественных новообразований головы и шеи является комбинированный метод. Однако показатели выживаемости по-прежнему относительно неизменны. хирургический этап лечения приводит к обширным дефектам, которые устраняют посредством реконструктивных технологий. Несмотря на то, что приоритет отдается одномоментным реконструкциям, до сих пор обсуждается проведение отсроченных реконструкций через 6–12 мес после хирургического лечения, в связи с чем необходима разработка новых методологических и практических подходов.</p><p><bold>Цель исследования </bold>– определение особенностей проведения отсроченного реконструктивно-пластического этапа у больных злокачественными опухолями челюстно-лицевой области, оценка возможных решений для оптимизации этапа.</p><p><bold>Материалы и методы. </bold>проведен анализ доступных источников литературы, опубликованных в базах данных Medline, Pubmed, eLibrary и др. Для написания систематического обзора были использованы 60 из 101 найденного исследования.</p><p><bold>Результаты. </bold>при проведении отсроченных реконструктивно-пластических операций днеобходимо учитывать возможность формирования в ходе реконструктивного этапа более сложных и больших по площади дефектов мягких и костных тканей. предшествующее хирургическое вмешательство и/или лучевая терапия создают значительные трудности в идентификации реципиентных сосудов. технологии компьютерной поддержки проектирования (computer-aided design, CAD) и компьютерной поддержки изготовления (computer-aided manufacturing, CAM) позволяют на дооперационном этапе спроектировать дизайн и позиционирование реконструктивного материала. Наиболее трудной является реконструкция нижней челюсти, что обусловлено необходимостью не только получения хороших эстетических результатов, но и восстановления биомеханики височно-нижнечелюстного сустава. Развитие специфических осложнений препятствует достижению главной цели реконструктивного этапа – улучшению качества жизни пациентов. так, в раннем послеоперационном периоде могут развиться некроз лоскута, тромбоз его микроциркуляторного русла, гематома. В позднем послеоперационном периоде одним из распространенных осложнений по-прежнему остается экструзия пластины. Важным аспектом планирования отсроченной реконструкции является профилактика или лечение остеорадионекроза. Ожидается, что сочетание предварительной углеводной нагрузки и даларгина в анестезиологическом пособии позволит добиться снижения частоты развития периоперационных осложнений и улучшения отдаленных результатов хирургического лечения.</p><p><bold>Заключение. </bold>Реконструктивно-восстановительное лечение пациентов с дефектами, требующими отстроченных реконструктивных операций, является сложной проблемой и требует разработки комплексного подхода с тщательным анализом имеющегося дефекта и предшествующей противоопухолевой терапии.</p></trans-abstract><kwd-group xml:lang="en"><kwd>delayed reconstructions</kwd><kwd>reconstructions of the maxillofacial region</kwd><kwd>personalized reconstructions</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>отсроченные реконструкции</kwd><kwd>реконструкции челюстно-лицевой области</kwd><kwd>персонифицированные реконструкции</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Bolotina L.V., Vladimirova L.Yu., Den’gina N.V. et al.
