The leading treatment is combined methods with surgery as a first step. However, operation is limited in a locally advanced process due to requires the use of complex reconstructive techniques and removal of aesthetic and functionally important structures. In this regard, the interest of specialists is understandable in use of organ-preserving techniques with comparable oncological results. Intra-arterial chemotherapy has shown high effectiveness in various localizations of malignant neoplasms, so its use in locally advanced tumors of the nasal cavity and sinonasal tract seems promising.</p><p><bold>Aim.</bold> To evaluate the results of complex treatment of patients with locally advanced cancer of the nasal cavity and sinonasal tract using induction intra-arterial chemotherapy.</p><p><bold>Materials and methods.</bold> The study included 28 from 2017 to 2023 with locally advanced cancer of the nasal cavity and sinonasal tract underwent TPF induction intra-arterial chemotherapy, followed by radiation or chemoradiotherapy. The primary endpoint of the study was survival, secondary – objective response rate, treatment toxicity and the possibility of organ preservation.</p><p><bold>Results.</bold> The median follow-up was 19.1 months. We obtained results for all patients of 1-year overall survival – 85.7 % (T3 – 100 %, T4a – 92.7 %, T4b – 55.6 %), and 1-year progression-free survival – 66.7 % (T3 – 75 %, T4a – 71.4 %, T4b – 50 %), respectively. The response rate after intra-arterial chemotherapy was 85.2 %. Overall survival and progression-free survival did not differ significantly between patients with complete response, partial response, and stable disease. However, in patients with a complete response after chemoradiotherapy, 1-year overall survival was 100 % (p = 0.009) and progression-free survival – 90 % (p &lt;0.001). The main side effect in patients in the study was the development of neutropenia (35.7 %), and occurrence of neurological complications was noted in 10.7 %. Of the 35.7 % of patients with tumor orbital invasion, survival with preserved visual function in the first year of life was 80 %; all patients retained vision; as a result of treatment, not a single patient underwent surgery with exenteration.</p><p><bold>Conclusion.</bold> Induction intra-arterial chemotherapy in treatment of patients with locally advanced cancer of the nasal cavity and sinonasal tract demonstrates high efficiency and a high level of complete responses with a fairly low level of toxicity, and also allows preserve the eye function for the majority of patients.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Злокачественные новообразования полости носа и придаточных пазух – редко встречающаяся патология, которая составляет менее 3 % всех злокачественных опухолей головы и шеи. Ведущим методом лечения этого заболевания является комбинированная терапия с хирургическим вмешательством на первом этапе. Однако при местно-распространенном процессе операция имеет расширенно-комбинированный характер – удаляются эстетически и функционально важные структуры, что требует использования сложных реконструктивных методик. В связи с этим понятен интерес специалистов к применению органосохраняющих методик при сопоставимых онкологических результатах. Регионарная внутриартериальная химиотерапия показала свою высокую эффективность при различных локализациях злокачественных новообразований, поэтому ее использование при местно-распространенных опухолях полости носа и придаточных пазух представляется перспективным.</p><p><bold>Цель исследования</bold> – оценить результаты комплексного лечения больных с местно-распространенным раком слизистой оболочки полости носа и придаточных пазух с применением индукционной регионарной внутриартериальной химиотерапии.</p><p><bold>Материалы и методы.</bold> В исследование вошли 28 пациентов с диагнозом «местно-распространенный рак слизистой оболочки полости носа и придаточных пазух», которым с 2017 по 2023 г. проведена индукционная полихимиотерапия по схеме TPF с внутриартериальным введением цисплатина и доцетаксела и последующим лучевым или химиолучевым лечением. Первичной конечной точкой исследования были показатели общей и безрецидивной выживаемости, вторичными конечными точками – частота объективного ответа, токсичность и возможность сохранения органа.</p><p><bold>Результаты.</bold> Медиана наблюдения оказалась равной 19,1 мес. Общая 1-летняя выживаемость всех больных составила 85,7 % (пациентов с заболеванием стадии Т3 – 100 %, Т4а – 92,7 %, Т4b – 55,6 %), безрецидивная 1-летняя выживаемость – 66,7 % (пациентов с заболеванием стадии Т3 – 75 %, Т4а – 71,4 %, Т4b – 50 %). Частота объективного ответа после внутриартериальной химиотерапии оказалась раной 85,2 %. Показатели общей выживаемости и выживаемости без прогрессирования значительно не различались у пациентов с полным, частичным ответами и стабилизацией заболевания. Однако у больных с полным клиническим ответом после окончания химиолучевого лечения 1-летняя общая выживаемость составила 100 % (р = 0,009), выживаемость без прогрессирования – 90 % (p &lt;0,001). Основными нежелательными явлениями были нейтропения (35,7 % случаев), неврологические осложнения (10,7 % случаев). У 35,7 % пациентов с инвазией опухоли в орбиту выживаемость с сохраненной функцией зрения на 1-м году наблюдения составила 80 %, при этом ни одному пациенту не потребовалось расширенно-комбинированное хирургическое вмешательство с экзентерацией орбиты.</p><p><bold>Заключение.</bold> Применение регионарной индукционной внутриартериальной химиотерапии в комплексном лечении пациентов с местно-распространенным раком слизистой оболочки полости носа и придаточных пазух демонстрирует высокие эффективность и уровень полных клинических ответов, довольно низкую токсичность и позволяет сохранить функцию органа зрения у большинства больных.</p></trans-abstract><kwd-group xml:lang="en"><kwd>induction chemotherapy</kwd><kwd>intra-arterial chemotherapy</kwd><kwd>squamous cell carcinoma</kwd><kwd>adenoid cystic carcinoma</kwd><kwd>nasal cavity and sinonasal tract cancer</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>индукционная химиотерапия</kwd><kwd>внутриартериальная химиотерапия</kwd><kwd>плоскоклеточный рак</kwd><kwd>аденокистозный рак</kwd><kwd>рак полости носа и придаточных пазух</kwd></kwd-group><funding-group><funding-statement xml:lang="en">The work was performed without external funding</funding-statement><funding-statement xml:lang="ru">Работа выполнена без спонсорской поддержки</funding-statement></funding-group></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Abdou R., Baredes S.
